Position Description and Responsibilities

Compassionate, Customer Service focused team member, with referral and intake experience, to assist the Home Health and Hospice Coordinator's in the day-to-day operations of the admission and referral team.  You will assist in ensuring the delivery of a quality admission and appropriate continuity of care throughout a patient's/family's transition from admission to creation of the Plan of Care. This competent and experienced health care professional should exhibit behaviors of accountability, flexibility, respect, and collaboration.

Skills / Requirements

  • Previous experience in the healthcare environment

  • Excellent customer service and communication skills

  • Excellent computer skills and working knowledge of Microsoft Word and Excel

  • Excellent time management and organizational skills

  • Self-directed, passion and commitment and able to solve problems independently, as appropriate
